- Birth Certificate.
- Adoption Papers or Legal Guardianship document (if applicable).
- If traveling unaccompanied or with someone other than a parent or legal guardian:
- A notarized letter from the parent(s) or legal guardian(s) confirming details of anyone accompanying the minor, and
- A copy of the parent(s) or legal guardian’s biographical page of their passport.
